Wiring is dedicated. Appears to be AT&T server problem.
Might go a day with no disconnects, then repeats.

That's why AT&T has a tech support department. Call them the next time it happens. It could be an intermittent in the modem, it could be a server problem, it could be a noisy line, it could be lots of things.
you do have the micro-filters that came with the modem plugged in between the phone socket and the wire going back into the modem don't you?

Try a different modem.

Check for the filter. Try installing a different filter. And check every phone jack at home. If there are devices connected on the same phone line, install filter on it.
Check the dsl light on your modem. If it is flashing green/red, it means that the modem is trying to get the dsl signal from the CO. Try calling AT&T technical support, it might be a line problem then.

You need to forward a port in your router, to your local "LAN" ip address. See http://portforward.com/ for more info.
You would also want to manually setup the LAN ip, atleast on the computer which you wish to use, because otherwhise it might change occasionally, and then the forwarded port would point to the wrong address.
Locate your router on this page, and follow the instructions.
